Overview

Are you looking for a position that utilizes your experience or degree in social services or criminal justice? Do you have case management experience of at least 1 year serving substance abuse or criminal justice populations? If so, we have an exciting full-time opportunity available at our Headquarters in Phoenix, AZ. Int his role, you will be is responsible for a case management caseload of program clients. Performs tasks required to screen clients relative to program eligibility requirements, conducts intakes and assessments on new clients, conducts ongoing interviews, develops client service plans, makes referrals for treatment, coordinates services provided to the clients, tracks, records, and reports on client progress. Serves, as a representative in court hearings and meetings with organizations external to TASC, Inc., that are relative to specific clients.

Responsibilities

RESPONSIBILITIES

 

  • Performs case management tasks necessary to scheduling and conducting interview, determining client program eligibility, screening of clients, client intakes and assessments, making client referrals for counseling, tracking/recording and reporting on client progress.
  • Maintains accurate and complete records of client caseload.
  • Schedules and monitors urinalysis testing.
  • Schedules and/or attends upon requests/subpoena court hearings and other meetings with other individuals/organizations relative to the client.
  • Attends scheduled clinical/management meetings for information exchange and case reviews.
  • Monitors TASC clients for urine sample collection where necessary.
  • Assists immediate supervisor performance of delegated/special duties.
  • Provides emergent coverage for absent program personnel.

QUALIFICATIONS

  1. Minimum Bachelor’s degree in Criminal Justice/Social Services or High School diploma or GED and 5 years of behavioral health experience.
  2. One year case management experience in a program serving criminal justice/substance abuse population preferred.
  3. Knowledge of substance abuse treatment and treatment community programs in Phoenix.
  4. Must have good verbal and written communication skills and the ability to effectively communicate to a wide variety of criminal justice and treatment professionals.
  5. Must be able to integrate knowledge with practice areas of criminal justice and substance abuse treatment in provision of service. Ability to perform a variety of case management tasks and duties within the framework stated TASC program and agency policy and procedures.
  6. Must pass urinalysis drug test.
  7. Must be eligible to work in the US for any employer.
  8. Must pass criminal background check.
  9. Must be able to obtain a Level 1 Fingerprint clearance card.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

 

  • Must be able to lift from floor to waist up to 20 pounds.
  • Must be able to read, write, and communicate fluently in English.
  • Must be able to give and receive verbal and written instructions.
  • Must possess the ability to utilize visual and auditory senses to complete all job duties as assigned.
  • Must be able to work in a typical office and healthcare environment.
